Required Documents

Recruitment in a Few Steps:

1. If you do not yet have an account on UczelniaOnline.WSNS - create one
2. After creating your account, click the activation link that will be sent to your e-mail address.
3. Log in to UczelniaOnline.WSNS go to the “My Studies” tab, select the type of recruitment you are interested in, complete the missing information about the school you graduated from, and register for the selected program.
4. Submit the required documents to the Recruitment Office (room 227, 2nd floor).

Candidates for First-Cycle (Bachelor’s) Studies and Long-Cycle Master’s Studies must submit the following documents:

• a printed and signed recruitment form, available after logging in to UczelniaOnline.WSNS and selecting the appropriate field of study,
• a copy of the secondary school leaving certificate (matura) (original for inspection) – scanned copies of matura certificates and other documents may be accepted without certification of conformity with the original; candidates will be required to submit original documents at a later date as part of the recruitment process - oświadczenie; prosimy o dostarczanie kompletu dokumentów drogą pocztową,
• 1 photography (meeting the requirements for issuing ID cards),
• identity card (for inspection),
• proof of payment of the recruitment fee (does not apply during promotional periods),
• a medical certificate issued by an occupational medicine physician confirming no contraindications to studying in the following fields:
  Emergency Medical Services, Cosmetology, Dietetics, Nursing, Dental Techniques, Physiotherapy,
• a signed educational services agreement in two copies (available after logging in to UczelniaOnline.WSNS and selecting the appropriate field of study).

Candidates for Second-Cycle (Master’s) Studies must submit the following documents:

• wa printed and signed recruitment form, available after logging in to UczelniaOnline.WSNS  and selecting the appropriate field of study,
• a copy of the Bachelor’s or Engineer’s diploma (original for inspection); for Nursing – a copy of the diploma confirming completion of first-cycle studies in Nursing (original for inspection),
• a copy of the diploma supplement (original for inspection) – does not apply to WSNS graduates,
• 1 photograph (meeting the requirements for issuing ID cards),
• identity card (for inspection),
• proof of payment of the enrollment fee (does not apply during promotional periods),
• a medical certificate issued by an occupational medicine physician confirming no contraindications to studying in the fields of Cosmetology and Nursing,
• a signed educational services agreement in two copies (available after logging in to UczelniaOnline.WSNS and selecting the appropriate field of study).

IMPORTANT:
For second-cycle studies in Nursing, only candidates holding a diploma of first-cycle studies in Nursing will be admitted.
